I believe that iems handle sound reproduction completely differently to full sized headphones since a full sized headphone allows the sound to interact with your external ear (the auricula and pinna). Since iems directs the sound straight into your ear canals it hasn't had the chance to properly interact with the human ear to lead to the sound that's more real or natural.
