Can I go straight out of a phono preamp into CIEMs? Not sure how I'd accomplish this, maybe some sort of specialized cable? or do I need a standalone amp as well?

Any simple headphone amp with low distortion that accepts RCA inputs will work fine. A JDS Labs Atom would be a fine pick.
