Completely under-rated. Also qualifies in the best double-album thread.

Does NIN's Downward Spiral count?

I think the second half of that album works amazingly well...

also Skinny Puppy's Last Rights, if you reburn a copy with 'Left Handshake' included as track 10.

Just came out yesterday-and I love it: Neil Young & Crazy Horse "Greendale."

Tommy is my second choice, but that came out when I was in high school and I have personally played it to death.

All these sugestions from 2003 are great,
Do you know any modern concept album, apart from the latest Porcupine Tree,
 
it seems they died with vinyl, the latest Arcade Fire is a gapless album but hardly a concept one,
smb here mentioned OK Computer, but I dont think its a true concept album,
 
Any suggestion?

I'd say my absolute favourite would be The Wall. I'd also give honorable mentions to Opeth's Still Life and any of The Mars Volta's albums.
 
Also, I know it's not supposed to be a concept album (by intention of the artist), but I'm also going to say Midlake's The Trials of Van Occupanther. 
 
Porcupine Tree's In Absentia is also a good one.

I'll 2nd the vote for "Thick as a Brick", for me that's the classic concept album.
 
As for more recent I really like Unitopia's "Artificial", it's one of my favorites from 2010.
