You just need a good cable. Depending on the person, varying degrees of memory wire might help. I personally like memory wire that wraps all the way around the ear. That keeps it feeling snug. Others might like no memory wire. If you had no over the ear style wire, the CIEm would be too heavy and would start slipping.

Some ciem manufacturers give the option of wearing the cable down.

I'm also looking for IEM with cables going downwards, as it is more comfortable and I am using my IEMs only sitting so I wouldn't need that isolation from cable shocks... 
Do you have names in mind ? I'm looking to resell my JH10X3 so one that would do resell aswell would be great !
