Old, from a wireless system. Not custom. A search brings up one place selling it with the receiver pack for $100 shipped. TRS mini plug listed. The body pack is stereo so it should work as a normal stereo plug. Looks to sit more on the outside of the ear canal and try to seal with some soft silicone and the sound comes through the hole and into the ear.
Got an extra $100 lying around? Doubt it is any better than okay sounding and the design might have been a bust fit-wise. Some DIY-er with extra cash could have fun trying to put sound tube on it. A bit of a chance but you never know.
I don't have much expendable cash right now myself but these are one of those "what the hell" items that make this hobby fun though.
