This charger outputs 2 amps (Nexus 7 tablet), my oneplus 3 charges fine, full charge in 2.5 hours. This charger is rated output 2 amps.

I use the charger which came with Asus nexus 7 2012 tab, which is a 2 amp charger, charges my oneplus 3 in office pretty well. So i think it is not the charger in my case.

This happened with me in v1.1 too, i started charging and aroung 91 it starts continous glow, i need to plug out tgen plug in to make it start blinking again. And is it so that after 90% it charges very slow.
