No, there are several custom IEMs that have a cable down option, but microphonics can be an issue with that configuration.  Some that I know of with a cable down option include the Starkey SA-12, Fabs Fabulous Earphones, and Minerva Mi-3.  You can read my reviews of custom IEMs here.
 
If you want ER4P performance with more bass, there is the TS842 with uses the same driver for mids/treble and a dynamic bass driver.  The SA-12 has a somewhat similar sound, but is much smoother, and moving up the UERM is a fantastic custom IEM that is along those lines.  The SA-43 can have a very flat sound compared with other custom IEMs, but someone in my thread moved from the ER4S to the SA-43 and has to adjust to the amount of bass, so what I am saying is something like the Q or anything else with enhanced bass will be a huge leap in bass quantity.  Bass in custom IEMs benefits from bone conduction as well as much better note sustainment, even is not enhanced.
