It is not an Altoids case. It is the zipper pouch which comes with Etymotic ER-6. It has 2 pockets, though I only use the back pocket to store the extra battery and Ety tools. If you put it in right, the whole thing can be zipped in, and the zipper comes up over the USB port, while the headphones and switch are still available. To change tracks or whatever, just zip it down a little and slip the Micro out from the top. I think it provides good protection, but the downside is that it is quite a squeeze to fit both the ER-6 and Micro in. Oh well, the headphones stay clipped to my shirt most of the time anyway.
You can order just the case for $15. Not too shabby, though compared to the altoids tin, this is relatively expensive.
I bet it is possible to use a thin strip of metal, wood or plastic to stabilize the belt loop on the back, thus turning it into a belt clip.
