Macbook Pro to Saffire Pro 40 via SPDIF Optical. Is it supposed to be this soft?

Hi all, I have been trying to use my Saffire standalone between my macbook pro and my Ultrasone headphone(monitor).
 
Macbook Pro -> Optical Out(Encoded Audio) -> ADAT(SPDIF Optical) -> Saffire Pro 40 -> Line Output 1 & Line Output 2 (one L one R) -> Y cable(mono to stereo) -> Ultrasone as monitor
 
I get very very very soft sound and wonder what I'm doing wrong here Saffire MixControl shows very healthy levels on the main mix out but the sound on the headphones is tiny even when the monitor level is turned all the way up.
 
Another thing is I can ONLY play encoded audio when my Audio MIDI Utility is open. Once I close that configuration, I lose lock and sound altogether. Could anyone shed some light one these issues? Am I doing the cabling right?
