Connecting wires from amp to 1/4" jack?

I'm trying to figure out how to complete this unfinished amp I bought (for headphones).  Everything is made except for the output, which I need to wire up.  The amp is similar to this:
 

 
 
I'm confused on how I wire up the + and - right channels and + and - left channels to a 1/4" jack.   That's 4 wires, so how do they connect to one jack? I understand how it would work for speakers, but not a singular thing.  I'm using an jack similar to this (on the right):

Tip left, Ring right, Sleeve ground.

okay, but does that mean i solder say both the + and - left to the tip, ie 2 wires from the amp board?  just twist them together and solder them to the tip?

No, left + to the tip, - to ground.
