Wiring info needed for headphone adaptor cable: 4-pin XLR female to 1/4-inch jack male

I have asked my local cable maker to build me a cable like the one shown in the links below:

Its function is to allow me to use my 4-pin XLR terminated headphones on a single ended amp without having to re-terminate my headphone cable.

My cable maker has requested wiring information for this cable. Can anyone tell me how it is wired please?

Most designs use this pinout:

Many thanks. Apologies if this is a daft question but how are these five connection points wired to the three connection points found on a 1/4" stereo jack?

You don't need to use the ground on the XLR.

Both - wires go to the sleeve

The R + goes to the Ring

L + goes to the tip.

I've made these adaptors before.
