YouTube audio extractor: recommendation needed

I have been using some web based services for this in the past but am wondering if there are better off line apps available for this.  I understand that youTube extraction is offering a very minimal sound quality (guess 128kbps or worse), but there is some really good Live music on there which I like to add to my library.
 
Also, the &fmt=18 for the high quality sound still confuses me....is this available for all vids or only HD vids?
 
In one of the older threads I saw dvdvideosofts' free Studio5 recommended, is this still the case?

I don't know about the quality but I use this http://youtubedownload.altervista.org/ the pro version. I haven't really used anything else to compare it with. The sound quality of music files created from it, seems very dependent on what was uploaded, its a bit hit and miss. Also just because something has HQ video doesn't always mean the audio is of the same quality.
