I spent a couple hours at FitEar's Chiba factory a few weeks ago and got to see more than one 335DW either queued to be built, or polished. I've written a little about the experience. BTW, I'm back from Hong Kong and will try to answer questions and pass them to Mr. Suyama if I cannot.

Yes and no -- You can order 335DW from authorized shops like Jaben, but you can't use ear impressions taken by random audiologist. The ear impressions has to be taken by FitEar-authorized audiologist or expert.

Yup, pretty much. Basically "available outside of Japan, but only where there are authorized FitEar CIEM retailers."
 
So people outside of Asia are out of luck.
