When I use my The "Caffeine Ultra" + UE Super.Fi 5 Pro EB I have this issue:
when I turn the amp on it doesn't play any sound. I must turn on and off a few times to get sound from the amp. Then it works just fine. Simply doesn't start properly when is turn on the first time (no sound). What seems to be the problem?

Broken amp? Do you have the same problem with other headphones?

No, only with these ones.

have you tried a fresh battery?

Yes I've tried. It seems to me that the amp when it starts doesn't have enough power to drive the earphones (very low impedance - 11 Ohms), and only when capacitor is fully charged starts to play...(I'm just gessing!)
