My er4p is currently of out warranty and sth just went wrong with the cable yesterday. I'm just wondering how am i supposed to send them my money, they give me a list of service fees as followed but i'm not sure which one it is before testing and evaluation by the company.

Includes testing, cleaning, new eartips, new filters and transducer replacement (one or both):

I would imagine you send it in and they will tell you how much it costs - then you pay by credit card or if you have a lot of time - send them a money order - I don't think they will take a personal check.

Yes, Etymotic would do a secure transaction with your CC information.  You might want to do the transaction over the phone.  
 
You might also ask if they have a PayPal address that you can very securely pay them that way.
