I found a place that does ear moulds.
Looking to get two pairs. One for use with Etymotic ER-15 Musicians' Earplugs. The other for in-ear headphones that I haven't chosen yet.

Does the audiologist attach the filters/headphones or is there a standard fitting? I suppose it would be cost-effective if the fitting were the same on the musicians' earplugs and the headphones.

Unless the audiologist is a dealer for whatever company's product you go with you get your moulds done by the audiologist and then send them of somewhere else to do your product. Some products (universal iem's etc) can use the same custom tips and others can't and I don't know about the ER15. If you are considering custom iem's they get build to your ear and there is no way you can use them as/on/with anything else.
