Going through my box of audio stuff and getting rid of the stuff I know I'm not going to use. This included both a stepped volume pot and input selector.
Volume pot: I think this was DIY stepped attenuator. If I recall correctly, it came in an exStatA that someone else built and I bought from them. The amp was set up for single ended input, but it looks like this attenuator can take a balance input (examine the photos yourself to verify -- I will not be liable if this is in error).
Input selector: I think I bough this to switch a balanced signal, but never got around to implementing it. Looks to switch 5 terminals through 8 inputs. Brand is Grayhill.
SOLD Price is $20 including USPS First Class shipping to the USA and PP fees. If you want expedited service, it will cost extra.
