Seriously struggling to get a good seal with IEMs

I recently got a pair of Shure SE-215 IEMs. I've tried almost all of the tips and the only one that even remotely seems to work is the small silicone tip - but even then it is quite loose and uncomfortable. All the foam tips simply slide straight oput. My left ear also seems to hold the earphones far less than my right. What can I do?

There are also Sony hybrid tips, JVC Spiral Dots, and SpinFit tips to try.
 
I find with Shures that Westone STAR tips offer the best sound, but that's as individual as the fit.

Well if you're getting the Westone STAR tips specifically they offer a set with a range of different sized tips so you can find which one fits you best. Otherwise medium or small might be better since you said small for the Shure tips fit you best.
