So if an amp has double 3.5mm outputs for the balanced mode, like the Sony PHA-3 or the Pono Player, and a headphone uses 3.5mm connectors, like the Sony MDR-Z7/Z1R...

Can you just pick up two 3.5mm cables and connect the right balanced port to the right headphone connector, and the left port to the left connector?

I guess the same question would apply for headphones with 2.5mm connectors, like the Nighthawk, as 3.5mm to 2.5mm cables are also readily available..?

Or do you need a specialty all-in-one balanced cable for some reason?

There's no reason that wouldn't work.  The only point of the all-in-one cable is the neatness factor.

You need to understand your devices first.

For any headphones use two wire (2 cores in each wire) as like HD650, you can cut out its ending terminal then weld new one to make it balance driving.
I dont know how is your Sony one.
After cut, identify as this : red core is R+, green core is L+ and two copper color is ground.

For amp output, research how manufacturer config their pin/poles. If they use two 3.5mm TRS (not the case Mojo where no balance output capable) may be Sleeve is for ground, Tip and Ring are for hot and cold signals and you need to use this pins. Left and Right side can detect easy. Each TRS 3.5mm is one channel
