Alright, I seem to have to replace the filters on my Blessing 2 Dusk every other day with Spiral Dots (other tips as well). I need some hints. The little sticky ring stays but the filter itself falls off. Today, I felt like it was stuck in my ear and went to the Dr....

PW Audio reportedly makes some good cables but would it really hurt them to use a metal splitter instead of the heatshrink on the less expensive cables? Charge a couple bucks more if they have to.

To reduce weight and the stress placed on the back of the ear.

Personally I've enjoyed cables with lighter Y-splitters / sliders made of wood / aluminium.

Don't use filters.
I know that they don't change the sound, but they do keep stuff out. Realistically, should I be worried about that?

It's just my opinion, but I don't think we should worry too much. A ton of IEM have similar nozzle designs and don't have anything to keep stuff out. Dunu SA6 is one that comes to mind.
I'd still be careful not to let stuff accumulate with the close proximity of the Knowles filter.

Unless you never ever clean your ears then it shouldn't be a problem. Maybe for some people who have a wet earwax condition that it could be a problem.
