OPA209 installed, black housing, very good condition. Includes USB cable -- though I'd recommend to try another one as this one sometimes seems to cut the connection when moving the unit (might be the connector in the amp as well -- did not check that).

Edit: checked with another cable, the amp seems all OK. The cable was obviously the culprit.
