Hum in left channel of Little Dot MK ii

All,
I just got a Little Dot MK ii off of Massdrop with CV 4010s.  I have a hum in the left channel that won't go away.  Here's what I've tried:
- a different AC cable
- a different AC outlet
- removed all vacuum tubes
 
If I remove the AC cable, the hum goes away while the amp continues to work for a bit as the caps die down.  If I plug the cable back in, the amp works fine for a few seconds, then I hear audible clicks and the hum comes back.
 
Anyone know what I can do here?  Presumably I don't need a ground loop isolator or new vacuum tubes because neither of those seem to be related to the problem?
 
Do I just have a defective amp?

I had a similar issue but it was to do with the source. Crappy laptop soundcard, becuase the hum disappeared with tablet/phone/mp3 player.
 
Also once, one tube was slanted just ever so slightly, and i straightened it, firm connection.
 
Dont know if that helps.

Its possible your amp is defective as well.

Yeah I think my amp is defective, but no one at MassDrop has responded to me about it.

Btw, when the bum is not happening, the amp will randomly click in the left channel.

Starting to get kind of annoyed... anyone on this site work at MassDrop that can help out? Or help diagnose this problem so I can open up the amp and fix it?

Hmm - how do I contact him?  There's no info on his website to contact him - and I'm not sure how I become a user on http://www.littledot.net/.
