How to make a robust detachable cable mod?

Folks, I have a pair of PX100s, two PortaPros, and one AKG K421 (my favourite, I love the way it sounds). All of them were broken at the plug, and so I tried to make a detachable cable mod for each of them. The thing is, what invariably happens is that it works well, then after two weeks or so the connection is fiddly again. It's always like this. ALWAYS. But I tug at the cables, and they're still firmly soldered to the female plug (I'm pretty sure the male to male cable isn't to blame). What am I doing wrong? I'm asking here because you guys seem like you do a lot of detachable cable mods yourselves, and I don't want to buy yet another pair of headphones, even if it does come with a detachable design by default.
