I need to way to mod the eartips for Ety's. Yesterday I had a little mishap with the flanges that wasn't good for me. Do I just take a razorblade and cut the whole first flange off?

How far do I cut?

Is there any other way that I could modify them?

Just turn your silicon tips inside out and insert them onto the earplugs backwards.
You now get the large flange to enter your ear first and thus get a really good seal.
