Help: loose mini plug

Hey guys,

I have an Etymotic ER4P that I've been using occasionally with this P-S adapter.

The thing is, the P-S adapter plug is a little loose when I plug it into my HEADSIX and pulls out pretty easily. Are there any good ways of getting it to fit more securely?

I wonder if it's loose at the tip or the barrel?

Did you try that adapter with other mini jack?
I think that most of of the mini plugs have the same size so it's hard to have it loose.
