Headphone Amp With Fixed Output

I am looking for a good headphone amp with a fixed output connection.  After doing some web searching, most of the headphone amps seem to offer only a variable output connection.  A combination amp/DAC would also be of interest.
 
Any recommendations would be much appreciated.
 
Budget is up to $1000.
 
Many thanks to anyone who can suggest a  candidate.

My Audio-GD is DAC/pre/headphone amp combo, with a button to switch to a fixed 2V/4V SE/balanced output on the back outputs. Why do you need fixed output? Most DACs are fixed, and it's easy enough to do with preamps anyways (same for headphone amps).

Some AudioGD DAC-HPamp units have a switch for the analog outputs on the rear, so they work as variable or fixed depending on what you need.
