I've never heard the fakes, but the second picture is the set I've always received from Amazon.

I've purchased a set over eBay for $11 without the case (yea, sounds totally legit!). I must say, if they are fakes, they are very good fakes, because I know I'm fairly sure I'd fail a ABX blind test between the EP630 and the CX300 (same driver IIRC) I bought from NewEgg (admittedly, the CX300 only costed $7.99 at the time with the headphone trade-in program, so if the EP630 was fake it'd have ended up costing more than the real thing LOL)
