Here is my first headphone amp build.  It's based on the TangentSoft tutorial and related articles.  It's using an OPA2132A and other than using bigger caps and a gain of ~5 it pretty much follows the "stock" Tangent design.  There's a couple issues (largely because of the big caps) but overall I'm happy with how it turned out.
