Perhaps my google skills are lacking but I don't see any balanced cables available for the Etymotic ER-4 range, is there a fundamental reason why this is not possible or is it just lack of demand (and possible connectors?).

I don't know where you could find the etymotic connectors to make a cable for them, but the stock cable could be modified to have a balanced connector on it.

If the Etymotic cable is maintains four conductors all the way down to the plug, then you just have to re-terminate it with the "balanced" connector you need for your amp.
