My audiologist made me hold open not without block (I think I was very still) and two minutes in she asked me to open and close mouth three times then another 2 minutes later she removed the impressions.

Does this sound fine?

I had my moth open about three fingers length.

My audiologist had me close my mouth all the time... He also had me sign the paperwork with the goo in my ear

I don't know what the correct procedure is; I guess you'll know if this worked once you get whatever it is that you went to an audiologist for (custom IEM's?)

If this is for ciems, then the procedure is whatever is listed on the chosen manufacturers site.  As a rule, it is open mouth with bite block for acryllic, and closed for silicone.  This is not always the case though.
 
To the op, if you are not sure of the quality of the impressions take a photo of them and send them to the company you wish to use.  They may or may not be able to tell if they are going to be ok.
