Figured it out.  Replaced the Q's in the right buffer 1 at a time.  It was Q4R.  Now have latch and all voltages check out.

Thanks cfcubed.  Have checked all the basic measurements short of putting in the tube.  Also, picks attached.  Seems like the problem is somewhere in the right buffer.  All suggestions appreciated.

Need some debugging help on my new build.  Start it up and get the red LED but it never latches.  Measured voltages at the R1E and R2E and get 12.06v and .712 respectively.  Any suggestions?   Other measurements:   OG to SG = 12.06 OL & OR are both close to 0 (3-5 mV)

Thanks John. So where did you connect the 2 input grounds? Also, my khozmo came with a wire bridge soldered between the middle and upper position for each output and ground position. Was yours different?

Thanks Sachu.  Well it's definitely related to the input wiring, because when I disconnect the input and input ground from each board the amp is dead quiet.

Grounding: right now, each board has 3 sg grounds (I'm using the alternate wiring scheme connection at the front of the board), that all connect to a central terminal.  That ground is isolated from the case (should it be?).  The IEC ground is connected to the case.     A bit of question mark...
