IEMs fall out of my ears if I use a tip that is too large. The opposite would also be true if your ear canal has a large diameter. Try smaller and larger tips, you might want to try out comply tips, too. Sometimes the foam just does a better job staying in than silicone flanges.

I don't have any real suggestions or improvements over what has been said, but somewhat related: when wearing glasses with headphones, I find it definitely worth-while to pull the ear-rests up above the headphones. It will tilt your glasses slightly, but it is much, much more comfortable, and...
