How much of a benefit would an external Amp/DAC offer over a Titanium HD?

I'm roughly 50/50 on gaming and music. I could deal with positional audio being slightly less in games, if there was a significant increase in music. I'm sure the EHP-O2D is better, but is it really £120 better?

I'm using a Sennheiser 598 at the moment.

With the ODAC, you would have no hardware positional audio in games at all, and it is only a better DAC than the Titanium HD if the latter is not performing properly because of some interference or other problem specific to your PC. In short: if your Titanium HD has no audible background hiss or interference noises, then it is very likely fine; in that case, it would make the most sense to get the O2 only. Because the HD598 is very sensitive to the output impedance of the amplifier, the O2 can make an audible difference (in particular, it will have less, but "tighter" bass), which may or may not be an improvement subjectively. If you can afford new headphones that the Titanium HD can drive reasonably well, and are better than the HD598, that would make the most improvement for the same amount of money.

I've actually compared a JDS Labs ODAC to my X-Fi Titanium HD.

 

The differences in sound are almost nonexistent. I really have to nitpick to find a difference once they're volume-matched for critical music listening.

 

It's certainly not worth it if you're gaming at all; I'd suggest buying a better set of headphones (and amp, if necessary) if a sound upgrade is what you want.
