After trying several options I've lost the stock supplied H-200 tips and gone for these Vmoda tips. I think the stock tips have to narrow bore using a larger bore helps open the presentation for me, lets the BA's breath a little.
This reminded me that I saw wide-bore tips somewhere in one of my drawers and I found them on the I-Digi freebies I got from Verizon a few years ago. Oddly I get much less flex when I insert these and am able to push them in a bit more so they seal up nicely. As you can see, they do have quite a wide opening (pictured next to the small, grey tips shipped with the H-200). A quickie search shows they have a few tip types available on amazon. For the last day or so of use they've sounded very good and having to adjust them less is a big plus. With a marginally deeper insertion I experience less of the relatively rare 'shouty' spot in the upper mids.
