Looking for DAC+Amp with Optical-in & maybe TRS balanced out

Hi,

 

I am looking for a primarily headphone amp for my PC that has the following features:

 

  1. Optical In (other inputs may be available, but it doesn't affect my decision)
  2. Independent power (obviously)
  3. Headphone jack (obviously)
  4. TRS balanced outputs with passive volume control on the unit (willing to give this up or have exchanged for non-balanced outputs, if balanced is too unreasonable a requirement)
  5. Switch on the unit to select headphones/speaker as output
  6. It will be very hard for me to cross $350 price tag

 

Does such a beast exist? I did some sleuthing around on recommendation lists on head-fi's main page, but nothing seems to match. USB/analog PC input seems to be the name of the game.

I've found NuForce HDP, but while it claims Optical in, the images they have of back panel only has coax. Any owners here that can comment? http://www.nuforce.com/hp/products/iconhdp/index.php
