filling out akg k701 warranty card.

the card has 4 blanks to fill out.

buyers name (easy)
dealer name (easy)
date (easy)
type number (???) what do i put there? the model? serial number? ect? and also where is it located is its something else.

sorry for being such a noob.
(first time with warranty card)

and also where can i get the address to mail it to.

Please enclose a note explaining the symptom or difficulties observed with the product’s performance. Also, include a contact name, the street address for return shipping and a daytime phone number should AKG need to contact you regarding your repair.
