You want the channels set to 2, sample rate set to whatever your files are (usually 44.1), analog out set to Headphones, SPDIF disabled, all other features under it disabled for music listening, and HF mode enabled on the right just to make sure everything's turned off. For games and movies, enable Dolby Headphone (I don't use the virtual speaker thing), set the sample rate to whatever the audio is (or just leave it at 44.1), and the number of speakers to whatever you can set the movie or game to use.

If this problem is exclusive to the DG, you have something enabled that you shouldn't have enabled. If this problem is not exclusive to the DG, it's a problem with the headphones and you might want to try something different.
