what has to be done to an HD650 to make it 'balanced'?

Balanced as in running it through a balanced amp.

 

If you stick a 1/4 inch jack-to-balanced terminal adapter onto the end of the HD650's stock lead - and put it into the balanced output of a balanced headphone amp running off a balanced source - are you now hearing a balanced signal through your HD650?

No, you don't want to do that.  Depending on how the adapter is made, its either going shorts the two negatives together, which may cause damage to the amp or use just one of the negatives. You need a balanced headphone cable.
