How to make an RCA to XLR Cable

I have a Little Dot DAC_1 and it has both RCA and XLR inputs.  I was under the impression that the RCA-XLR cable that I bought off of monoprice would work fine, but all I get is some static and very poor quality audio.  I then saw a post on a different forum saying that they were incorrectly wired.

 

So, I'm planning on disassembling them and rewiring them, but I'm not sure how they are supposed to be wired up.

 

Also would I be better off using the XLR or RCA outputs for the best sound?  Ones going to be going to my headphone amp and the other is for my speakers.

You DAC has balanced outputs, is your amp balanced?

Thanks, that chart gave me pretty much all the information I need and I got them rewired correctly.
