Definitely the customs are going to win out: the custom fit changes the game completely. However, it's a different question to see if the additional cost is justified...

There are alot of customs maker, and there demo unit have to make universal so anyone can try but there is different when come to customs. There are several customs maker release customs iem universal made, use their customs iem multiple ba driver into universal shell.

At the beginning, it was said that the ba200s have replaceable filters...  I'm curious, with IEs that have these type of filters, do they last forever?  How do they affect the sound.  I'm just concerned, because I've used my last IE for over 3 years.  If something happens, I doubt the filters will always be available to purchase, and so I worry if it affects the sound...

It depends on the way you use your IEMs. If you never maintain them, then of course those filters will get clogged easily. The main goal of the outer filter is to trap the cerumen and dust that may accumulate so that the inner acoustic damper doesn't get affected. However, TDK only supplies a finite number (~6) of filters for you to replace. Luckily the plastic acoustic damper is indeed replaceable. The ones that TDK uses are likely from Sonion because they're plastic (Knowles only makes metal-rimmed ones), but if you can find one that fits, you can possibly replace it as often as you like. Acoustic dampers are only a couple bucks a pop, though you'll have to dig to find them, as they're usually specialty items for use in hearing aids.

How fussy is it getting these into your ears and having them stay there? I had to get rid of my r-50 because it was too much hassle to get them in my ear. Also, I have a lot of trouble with the Sony MH1C's flat cord staying put in my ear -- the cord wants to stay straight and tends to push the buds out.

I don't think its hard at all but I also think getting the R-50's to stay in was very easy.  Why do you have so much trouble with the fit of IEM's, are they really loose or to tight?  For me I've never had any fit issues with any IEM well the TF10 I had some trouble but after I did the flip flop mod it was perfect.  I think the BA200 fits great and sits right inside your ears perfectly but the cable from time to time and mainly when jogging falls off but I don't use the chin clinch though.

I generally don't have much of a problem. Pairs of shures and klipschs I had a few years ago fit fine, as do a pair of 8320s and ckm 500s. The R-50's memory wire was the problem for me -- really irritating. I think it would've been fine otherwise -- unfortunately I liked the sound quite a bit. The problem with the MH1C's for me is the flat cord + the j-wire don't have enough give for me, so it forces the bud out of my ear. I do use my IEMs in the gym at times, and other times I'm forced to take them in and out, so ease-of-fit and need for mobility is key for me.

Right now I'm looking at:

Brainwavs B-2
GR01
BA200

The thing I most want is 3-D imaging + soundstange, but I'm afraid that the BA200s will be too warm for me (frankly I'm not wild about the CKM-500s) and that the fit will be an issue. The problem is that I haven't seen an IEM that's been described as leaning analytical with 3-D imaging + wide/deep soundstage that's in my price range.
