The ultimate sad song:  Gloomy Sunday.  It has been known to tip people over the edge.

My only son - Duncan Browne.  Or Fruit Tree by Nick Drake.

Fruit Tree has some of the best lyrics ever.  Basically talks about what would happen to him after he died.  Nick Drake is amazing.   Some people on these types of forums are nuts though.  Some of the stuff they post isn't sad at all.  Shows how much they've heard.   Nick Drake, Judee Sill, Early 70's Elton John, Fotheringay by Fairport Convention.  I could go on for awhile.

Not the saddest, but moving nonetheless: http://www.youtube.com/watch?v=o22eIJDtKho (Johnny Cash - Hurt)

Not a song, but part of a symphony. It's the 4th Movement of Tchaikovsky's 6th Symphony. I've always considered that to be one of the pinnacles of sadness expressed through music.
