1/4 to 1/2 problem

I have a headroom amp with 1/4 in. output but i have a headphone cord that is 1/2 in. so I am forced to use a adaptor that is of high quality but it cuts out constantly ,gets loose is there anything that will make this more stable?

thanks

Is the phone plug pulling out from the adaptor or is the adaptor pulling out from the amp? I had a similar problem with My Grado 1/4 to mini adaptor when it turned a certain way it would cut out the left channel or crackle. It was due to a slightly mishapen mini plug, the way it sat in the plug itself. That might be it or You have a wire problem in the adaptor itself.
