Is there any way to insert a VST effect into the regular Windows 7 audio chain so that sound from all applications pass through it? I guess it probably would have to be done via sound drivers, but maybe there is another way?

Virtual audio cable is one possible solution.
Another way to go is hardware depend where you use a loop-back cable (would maybe be better through digital path than through analog path). Many audio interfaces (and some integrated audio chips) supports two or more separate audio output streams
- loopback cable from output into some line input
- set the system output through headphones port
- set your VST hosting software take it's input through line-in and output through std. outputs
