Thank amb, it all works great now!

Now I need to figure out what polarity the charger jack has. I'm hoping it's the standard tip is positive?

See the Parts list section for the wallwart. Also if you read the schematic it should be apparent which polarity is used for what.

Replaced my opamps with a new ARDZ from Digi-key and OPA690 from amb, all good now. Had to try a few times at reflowing U5 pin 7 to get normal DC offset in the left channel. Man, this thing sounds nice.
