The board calls for a 24V or 30V secondary with center tap, or with two separate 12V or 15V secondaries connected in series. Your transformer has neither of these configurations.

May I ask which kit you bought? I'm looking for a speaker protection kit myself and don't recognize the schematic you posted.

It's a kit from Vintage Audio Labs in ebay

Top Quality Speaker Protection Module for Audio DIY - eBay (item 300387316867 end time Feb-14-10 13:06:22 PST)

A member of audiokarma told me that with just one 12v input the kit will work
