Does anyone have any schematics for an old Pete Millett hybrid amp design using a BUF634?

I've heard that there's an old Pete Millett hybrid amp that usese the BUF634 for the buffer. Does anyone have a schematic?

This one is the revMH Millett Hybrid, modified/refined by drewd and n_maher - it can use the BUF634, the OPA551, or the HA3-5002:

Pete's original can be found on his website - it was also published in the November, 2002 edition of AudioXpress magazine. It uses the 5-pin TO-220 version of the BUF634 on heat sinks:
