I know you are a self confessed newb, but really you should try to do a little investigating. That's a big part of DIY, finding the info and teaching yourself about what it is you are trying to do. Yeah, it is basically a board that needs a box and a trafo, but what about learning how to put in some upgrades, maybe some caps, and the like.

Anyway make three wires out of the four by tying the commons together, on the 12v you can leave one pair open and unattached or parallel them. Use a meter to figure out what is what for the leads and look at the data sheet too.
