Is Auzentech the only other brand that makes X-fi cards?

Yes. The X-fi chips are licensed to Auzentech for use on their cards, such as:
Prelude 7.1
Forte 7.1
Home Theater HD 7.1 (upcoming)

No other company has rights to use the chips in their designs.

I have the list for:

Revolution 5.1
Audiotrak prodigy HD
Asus Essence
Auzentech X-fi forte
HTOmega Halo

Are there any other sound cards that can do 5.1 sound, AND have a headphone amp on them?

External like USB/firewire is fine.

the Revolution 5.1 and Prodigy HD don't have a technical "headphone amplifier" stage

however any audiocard with opamps can drive headphones, as the Revo and Prodigy do

and many external solutions are targetted at studio style use, and most all of them have some form of headphone stage
