Second when I measure the voltage at each leg, I getting 4.95V and 2.57V which added is the same as the battery V when its connected to the PS. However each leg is far off....

Even when I attached an external 12V DC PS, I was getting ~10V & 2V on the legs. Is the TLE damaged? Should I have the Caps before the TLE in the circuit. I've wired it just like the schematic in my first post.
You've got something miswired... the TLE should not get hot, and you should wind up with a voltage split within 10mV or so of one another.

Updated the OP, build is done and I'm satisfied for now... I'm already contemplating building a better amp, though my question is what's the next step?

For the near future I have no plans to get into using headphones, I'm primarily stuck on using IEM's (RE2 currently) and shortly thinking of upgrading to RE1's, maybe.

What would be the next big step in amp choices, I'm really liking the Starving Student Millet amp... have always been fond of tube amps. Since I'm only using IEM's, will I see any benefit in using higher-end amps or those reserved for powering high-impedance headphones/speakers? Would my time be better spent building a DAC since I'm currently powering this of my MacBook Pro, which I'm sure isn't great (sound card).
