IEM extension cable

Hi

I want to make custom IEM extension cable (small jack in - small jack out). What I have problems finding is a good quality cable to use in my setup.
I do not want anything large/thick... a cable similar to the one used in Shure/Westone IEM's would be perfect.

Any hints/tips welcome ?

Why not just buy one of the manufactured IEM extension cables? The wire is thin and the terminations will be less bulky.
